How did the Nazis conceal the Holocaust so effectively?

While there were some reports of ongoing genocide before 1945, the Allies were largely ignorant of the full extent of the Holocaust until they reached german mainland and liberated the concentration camps.

Considering the intelligence leaks and spies that the Nazis suffered, how were they able to keep such a giant operation almost entirely unnoticed?
