Do I get back with him?

Simple question. I broke up with him due to just not being present and reciprocating nothing. We both (22 m/f)put a lot into the relationship the first few months but it just dwindled when life got too busy. We both got promotions, I was working 80 hour weeks. We barely had time, barely had sex.

The last night we saw each other, I wound up in hospital with no one and he hadn’t messaged me for over 36 hours. Not crazy but a few weeks prior I was up from 2am till morning and had a shift just to be there for him in hospital. So that day really opened my eyes. I am still madly and idiotically in love with him. I just need some tough love and past experiences from you guys. I want to give him another chance but at what cost? He is my first everything, and he was there for me until the end.
