What did I do wrong with the Shinkansen Ticket?

Hi all,

I have been in Japan 2 years ago and will fly again next month. Something just came into my mind, which I am curious about what I did wrong.

I took the Shinkansen from Osaka to Tokyo. So I obviously used my Shinkansen Ticket which I picked up beforehand to get to the right Track and took the Shinkansen. When I arrived in Tokyo, I was not able to use my Shinkansen Ticket to get out of the Station. I tried it out, tried out my Suica, nothing worked. I had to ask the Staff there to let me out, i can't remember exactly what they did. I think they did some adjustment on my Shinkansen Ticket which I was then able to get out.

Is there anything I maybe did wrong? This year I will be taking multiple Shinkansen, so I want to be prepared and not waste time.
